  • Email, telephone, and in-person communications

  • Through our practice management system (Halaxy)

  • With your consent, from other healthcare providers involved in your care

We only collect information that is relevant to your treatment and will always explain why it is being requested and how it will be used.

Third Parties

Where reasonable and practicable, we collect your personal information directly from you.
However, there may be occasions where information is provided by other health professionals (e.g. GPs, specialists, or psychologists).

Access to Your Information

For your protection, we may ask for identification before releasing records.
We aim to respond to requests within 14 days.
